目的:分析上肢康复机器人治疗脑卒中偏瘫患者上肢功能障碍的疗效。方法:以 2020 年 6 月—2021 年 12 月在吉林大学第一附属医院康复科接受治疗的 42 例脑卒中伴上肢功能障碍患者为研究对象,随机分为观察组(21 例)和对照组(21 例)。两组患者均接受常规康复治疗,观察组患者在常规康复治疗的基础上接受上肢康复机器 人治疗,连续康复训练共 4 周。于治疗前、治疗 2 周、治疗 4 周后采用简化的上肢 Fugl-Meyer 评分法(Fugl-Meyer Assessment,FMA)、改良的 Barthel 指数(Modified Barthel Index,MBI)和改良 Ashworth 量表(Modified Ashworth Scale,MAS)分别评估两组患者的上肢运动功能、日常生活活动(Activities of Daily Living,ADLs)能力和肱二头肌 的肌张力程度。结果:治疗 2 周后,两组患者的 FMA、MBI 和 MAS 评分均较治疗前改善,差异有统计学意义(P<0.05), 且观察组患者 MAS 评分显著低于对照组,差异有统计学意义(P<0.05),而两组患者间 FMA 和 MBI 评分无显著差 异(P>0.05);治疗 4周与治疗 2周后比较,两组患者的 MAS、FMA和 MBI评分均有改善,差异有统计学意义(P<0.05), 且观察组各项评分显著优于对照组,差异有统计学意义(P<0.05)。结论:上肢康复机器人训练能有效地提高脑卒 中偏瘫患者的上肢运动功能和 ADLs 能力,且能有效降低肱二头肌的肌张力。
Objective: To analyze the efficacy of upper limb rehabilitation robot in the treatment of upper limb dysfunction in stroke patients with hemiplegia. Methods: A total of 42 patients with upper limb dysfunction after stroke hemiplegia admitted to the Rehabilitation Department of the First Hospital of Jilin University from June 2020 to December 2021 were randomly divided into the observation group (21 cases)and control group(21 cases). The patients in the two groups received routine rehabilitation treatment, and patients in the observation group received 4-week upper limb rehabilitation robot-assisted treatment on the basis of routine rehabilitation treatment. The upper limb motor function, activities of daily living (ADLs) and muscle tension of biceps brachii were evaluated by simplified Fugl-Meyer Assessment(FMA), Modified Barthel Index (MBI) and Modified Ashworth Scale (MAS) before treatment, 2 weeks and 4 weeks after treatment. Results: After 2 weeks of treatment, the scores of FMA, MBI and MAS in the two groups were significantly improved compared with those before treatment. The MAS scores of the observation group were significantly lower than those of the control group, and the difference was statistically significant (P<0.05), while there was no significant difference in the FMA and MBI scores between the two groups (P>0.05); Compared with patients treated 2 weeks, the scores of MAS, FMA and MBI of the two groups of patients treated 4 weeks were all improved, and the differences were statistically significant (P<0.05), the scores of FMA and MBI in the observation group were higher than the scores in the control group (P<0.05), and the score of MAS was significantly lower than that in the control group (P<0.05). Conclusion: Upper limb rehabilitation robot-assisted training can effectively improve the upper limb motor function and ADLs ability of stroke patients with hemiplegia, and can effectively reduce the muscle tension of biceps brachii.
