机器人辅助腹腔镜合并阴式手术治疗外阴鳞癌一例报道

外阴癌是较罕见的妇科恶性肿瘤之一,发病率占女性生殖道恶性肿瘤的 3%~5%,常见于绝经后妇女。 2021 年 2 月武汉大学中南医院收治了1例老年女性患者,因“外阴恶性肿瘤第 3 次化疗后”入院治疗,行“机器人辅 助全子宫切除 + 双侧附件切除 + 外阴癌根治术 + 腹腔镜双侧腹股沟淋巴结清扫术”,术后病理结果确认为外阴鳞状细 胞癌(高分化),随访未见明显局部复发及转移,取得了满意的结果。该手术的实施进一步推动了达芬奇 Xi 机器人手 术系统辅助腹腔镜在外阴癌手术中的应用,但其安全性、有效性及远期疗效需要进一步随访及扩大样本量进行探讨。

Vulvar cancer is one of the rarest gynecological malignancies, which accounts for 3%~5% of female reproductive malignancies and commonly found in postmenopausal women. In February 2021, an elderly female patient who was admitted to Zhongnan Hospital of Wuhan University due to vulvar malignant tumor after the third chemotherapy. Robotassisted hysterectomy+bilateral salpingo-oophorectomy+radical resection of vulvar carcinoma+laparoscopic bilateral inguinal lymph node dissection were performed. Vulvar squamous cell carcinoma was confirmed by postoperative pathology results (highly differentiated), and no obvious local recurrence or metastasis were observed in follow-up. The clinical efficacy of Da Vinci Xi robot-assisted laparoscope in treatment of vulvar cancer was further confirmed by this report. However, further follow-up and larger sample size are needed to confirm its safety, effectiveness and long-term efficacy.
