南京中医药大学附属医院泌尿外科 2021 年 4 月收治 1 例左侧孤立肾合并肾透明细胞癌患者,术前行 MRI 检查提示患者同时合并罕见的双肾盂双输尿管畸形变异。经术前充分评估,考虑患者解剖变异较大,为避免术 中重复肾盂及输尿管损伤,笔者为患者行经腹膜后入路机器人辅助单孔腹腔镜手术。术中充分游离左肾动脉,精准 阻断肾下极供应血管,行根治性肾部分切除术。手术进展顺利,手术时间为 80min,术中出血量为 30ml,无围手术 期并发症发生,患者术后顺利出院。孤立肾双肾盂双输尿管重复畸形合并肾透明细胞癌病例罕见,需进行充分的术 前影像学评估。经腹膜后入路的机器人辅助单孔腹腔镜手术中解剖精细,创伤小,术后恢复快,是临床可行的手术方式。
A case of solitary kidney with renal cell carcinoma in left kidney was admitted to the department of urology of Affiliated Hospital of Nanjing University of Chinese Medicine in April 2021. Preoperative MRI examination showed malformation of double renal pelvis and ureters. To avoid repeated injury of renal pelvis and ureters during surgery, robot-assisted single-port laparoscopic surgery with retroperitoneal approach was performed. During the operation, the left renal artery was fully dissociated, the supply vessels of the lower pole of the kidney were precisely blocked, and radical partial nephrectomy was performed. The surgery was completed smoothly, with an operation time of 80min and blood loss of 30ml. No perioperative complications were found. The patient was successfully discharged. Double renal pelvis and ureter malformation of solitary kidney with renal clear cell carcinoma is rare, which needs adequate evaluation based on preoperative imaging. Robot-assisted single-port laparoscopic retroperitoneal surgery is clinically feasible to treat renal cell carcinoma with small trauma and quick recovery.
