目的:探讨基于加速康复外科(ERAS)理念的护理模式配合康复机器人辅助训练在重型颅脑损伤术 后患者下肢康复护理中的运用效果。方法:选取 2020 年 1 月—2022 年 12 月就诊的重型颅脑损伤术后患者 186 例, 按照随机数表法将其分为对照组(93 例,行常规护理配合常规训练)和研究组(93 例,行基于 ERAS 理念的护理 模式配合康复机器人辅助训练),比较两组患者下肢肌张力、运动功能、平衡功能及并发症发生情况。结果:与干 预前比较,两组患者下肢各肌群的改良 Ashworth 量表(MAS)评分降低,与对照组比较,研究组 MAS 评分更低。 与干预前比较,两组患者干预后功能性步行量表(FAC)分级、简式 Fugl-Meyer 量表(FMA)评分、Berg 平衡量表(BBS) 评分升高,与对照组比较,研究组 FAC 分级、FMA 评分、BBS 评分更高。研究组并发症发生率低于对照组。结论: 基于 ERAS 理念的护理模式配合康复机器人辅助训练可改善重型颅脑损伤术后患者下肢肌张力、运动功能以及平衡 功能,降低并发症发生率。
Objective: To explore the effect of nursing model based on the concept of enhanced recovery after surgery (ERAS) with rehabilitation robot-assisted training in the lower limb rehabilitation nursing care of severe traumatic brain injury patients with after surgery. Methods: 186 patients after surgery for severe traumatic brain injury in the First Affiliated Hospital of Air Force Military Medical University from January 2020 to December 2022 were selected. They were divided into the control group (n=93) and the study group (n=93) using a random number table. The control group received routine care and rehabilitation training, while the study group received ERSA nursing model combining with rehabilitation robotassisted training. Lower limb muscular tension, motor function, balance and complications of the two groups of patients were compared. Results: Compared with that before intervention, the Modified Ashworth Scale (MAS) scores of patients in the two groups were lower, and the MAS scores in the study group were lower than those in the control group. Comparing with those before intervention, the Functional Ambulation Classification (FAC) rating, the Fugl-Meyer Assessment Scale (FMA) score, and the Berg Balance Scale (BBS) score of patients in the two groups were higher after intervention, and they were higher in the study group than the control group. The complication rate of the study group was lower than that of the control group. Conclusion: ERSA nursing model combining with rehabilitation robot-assisted training in lower limb rehabilitation nursing can improve lower limb muscle tension, motor and balance function of patients after surgery for severe traumatic brain injury, and lower the incidence of complications.
