目的:探讨基于加速康复外科(ERAS)理念的延续护理配合平衡障碍康复机器人在下肢骨折患者中的运用。 方法:本研究选取 2020 年 3 月—2023 年 3 月空军军医大学第二附属医院收治的 102 例下肢骨折患者作为研究对象,按照随 机数字表法分为试验组和对照组,每组 51 例。对照组采用常规护理模式干预,试验组采用基于 ERAS 理念的延续护理配合 平衡障碍康复机器人干预。比较两组患者的手术指标、焦虑抑郁评分、创伤严重程度及预后、术后各时间点疼痛评分以及并 发症发生情况。结果:试验组患者术中出血量、术中输血例数、病灶愈合时间及总住院时间显著低于对照组(P<0.05)。干 预后,两组患者焦虑自评量表(SAS)评分,抑郁自评量表(SDS)评分,创伤严重程度评分(ISS),急性生理学及慢性健 康状况评分系统Ⅱ(APACHE Ⅱ)评分,术后 2 h、6 h、12 h、24 h 视觉模拟量表(VAS)评分均低于干预前,且试验组显 著低于对照组(P<0.05)。试验组并发症总发生率显著低于对照组(P<0.05)。结论:基于 ERAS 理念的延续护理配合平衡 障碍康复机器人在下肢骨折患者中的运用效果显著。
Objective: To investigate the efficacy of enhanced recovery after surgery (ERAS)-based continuing nursing care combined with the balance disorder rehabilitation robot in the management of lower limb fracture patients. Methods: 102 lower limb fracture patients who were treated in the Second Affiliated Hospital of Air Force Medical University from March 2020 to March 2023 were enrolled. They were randomly divided into the experimental group (n=51) and the control group (n=51) using a random number table. The control group received routine nursing intervention, while the experimental group received ERAS-based continuing nursing care combined with the balance disorder rehabilitation robot intervention. Surgical indicators, self-rating anxiety scale (SAS) scores, self-rating depression scale (SDS) scores, injury severity score (ISS), acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) score, visual analog scale (VAS) at 2 h, 6 h, 12 h and 24 h, and complication rates were compared between the two groups. Results: The experimental group showed significantly lower intraoperative blood loss, fewer intraoperative blood transfusions, shorter fracture healing time, and reduced total hospitalization duration compared to the control group (P<0.05). After intervention, both groups showed decreased SAS, SDS, ISS, APACHE II, and VAS scores at all postoperative timepoints compared to those before intervention (P<0.05), with the experimental group demonstrating superior reductions (P<0.05). The total incidence rate of complications in the experimental group was significantly lower than that in the control group (P<0.05). Conclusion: ERAS-based continuing nursing care combined with the balance disorder rehabilitation robot can significantly improve clinical outcomes in lower limb fracture patients with reduced perioperative risks, enhanced recovery, and favorable safety.
